    All students entering North Carolina Public Schools for the first time must present proof of immunizations and have a health physical completed, within the past year, on the North Carolina Health Transmittal Form. Proof of immunizations must be official records with the student's name, date of birth, listed immunizations received, the date the immunizations were administered, and stamped by the doctor's office. Both the physical and immunization records are due by the first day of school.

    Kindergarten Immunization Requirements



    **Proof of immunization and physical must be provided within the first 30 days of school.  If you do not provide this information within 30 days from their start date, your child will be unable to attend until proof of immunization and physical is provided.  Doctor's appointments usually fill quickly so try to schedule your appointment before July to make sure you are ready for the start of the school year.  Please contact me if you have any questions.
