    Kindergarten Students

    Proof of immunization should be turned in with physical form.  This form can be found on the important links page or picked up in the nurse's office.

    Important Information for Parents with Children Entering Middle School

    New Immunization Requirements Beginning July 1, 2015:

    Children who start 7th grade on or after July 1, 2015, will be required to have vaccines recommended by

    the Centers for Disease Control and Prevention (CDC) to protect them from serious diseases. Students

    entering 7th grade will be required to have one dose each of Tdap (tetanus, diphtheria, pertussis) and

    meningococcal vaccines if they have not previously received these vaccines.

     

    2020 Recommended Vaccinations for Children (7-18 years old)

     

    **Proof of immunization must be provided within the first 30 days of school.  If you do not provide this information within 30 days from their start date, your child will be unable to attend proof of immunization is provided.  Doctor's appointments usually fill quickly so try to schedule your appointment before July to make sure you are ready for the start of the school year.  Please contact me if you have any questions.
