    Medication Orders:

      Please note that all students requiring medication adminstration by school staff will need a new "Permission to Administer Medication" form completed and signed by their physician and parent prior to any medication administration. This includes, both prescribed and over-the-counter medications.

      Orders are only valid for the current school year and must be updated at the beginning of each new academic year. 

      Medication changes require a new doctor's order form to be on file. This includes any change (amount, time, etc.).

    Medication Check-In/Check-Out:

    ALL medications are to be brought in by the parent/guardian and checked in with the nurse. There will be a process of signing medications in and out. Medications will need to be picked up/signed-out at the end of the school year.

    Presciption medications: must be in original bottle or box with clearly legible and current RX label (student's name, ordering amount).

    Over-the-counter medications: must be brought in the original bottle/box and unopened.

    **Medications will not be accepted in unlabeled or mislabeled containers.
