    Medication Orders:

Please note that all students requiring medication administration by school staff will need a new Currituck County Schools "Permission to Administer Medication" form completed and signed by their Physician and Parent prior to any medication being administered at school. This includes both prescribed and over-the-counter medications.

    •   Orders are only valid for the current school year and must be updated at the beginning of each new academic year.
    •   Medication changes require a new doctor's order form to be on file. This includes any change (amount, time, etc.).

    Medication Check-In/Check-Out:

ALL medications are to be brought in by the parent/guardian and checked in with the nurse. There will be a process of signing medications in and out. Medications will need to be picked up/signed-out at the end of the school year.

Prescription medications must be in original bottle or box with clearly legible and current RX label (student's name, ordering amount).

Over-the-counter medications must be brought in the original bottle/box and unopened.

**Medications will not be accepted in unlabeled or mislabeled containers.
