The NC Health Assessment form is required for students who are entering Kindergarten or are new to NC Schools and must be completed by the 30th day of school per NC general statute.
Per NC general statute, immunizations are required for entry to Kindergarten, 7th grade, and 12th grade and must be done by the 30th day of school. Please be sure to bring in proof of the required immunizations for your child.
