This form is to be used for students entering Currituck County Schools.

This form is to be completed by the parent/guardian when the family is sharing housing due to hardship.  It must be accompanied by the Shared Housing Affidavit - Property Owner form found on this list.  Documentation may be required to support the reason for shared housing.

This form is only used if parent/guardian is living with someone in a shared housing situation.  It is to be completed by the property owner and submitted (with supporting documentation) along with the Shared Housing Affidavit- Parent/Guardian form completed by the parent/guardian.
