Currituck Internal Medicine and Family Practice will hold Athletic Physicals on Tuesday, June 28 at Currituck County High School. Spots will be limited this year and will fill up fast. Please read the following instructions to reserve your spot.
- These physicals will be done for rising 9th graders to 12th graders. (High School Athletes Only!)
- Please complete pages 1,2 and 4 on the link below.
- These completed pages must be dropped off at Currituck Internal Medicine (534 Caratoke Hwy, Moyock, NC 27958) by 12:00pm Friday June 24th .
- They will review/prescreen these forms and tell you what time to be at the High School on June 28th or will put you on a waitlist if there are cancellations.
- Please provide/include any documentation or clearance letters from other doctors for known conditions
- The cost this year is $15.00.

NextCare in Elizabeth City will do physicals for $25 if your child's physical has expired or you would like one before June 28th. We apologize for the limited number, but we value everyone's time and want this to be as safe and efficient as possible.

CCHS Athletics
Welcome to CCHS Athletics, "Home of the Knights".CCHS has excelled in academics and in athletics. The CCHS athletic department supports many varsity sports. We belong to the Northeastern Coastal Conference. Our athletic program emphasizes activities where participation by as many student athletes as possible is a primary objective. CCHS has set high goals and standards for our programs. All of our programs are guided by the rules and regulations of the North Carolina High School Athletic Association (NCHSAA). We expect our athletes, coaches and parents to always represent CCHS and the community with the utmost respect as well as adhere to all school and district policies (see the handbook for more details). **CCHS won the Wells Fargo Cup in 2012, 2013, and 2015 for having the best overall athletic program for that school year. We also won the Northeastern Coastal Conference Sportsmanship Award for 2016-2017. Both of these awards are a tribute to our coaches, players, administrators, staff, parents, and fans......a true TEAM effort!
